The PHCD Community Health Model

The past few weeks have been a whirlwind for PHCD Board Members, staff and partners. While immersed in our work in securing an new quality operator for Petaluma Valley Hospital, we've been honored to receive awards from our incredible local agencies, recognized in news and industry publications, and asked to address our community and our colleagues at variety of conferences and events. In particular, our efforts to collaboratively address the root causes of poor health have been celebrated and lauded. Our spirits are lifted by these acknowledgements, and we wanted to a take a few moments to share them with you:
On November 7, the North Bay Business Journal published an excellent article on PHCD's efforts to ensure access to a quality hospital and acute care services as well as our collaborative efforts to impact health outside the walls of our hospital.
Champions for Children
On November 3, our fearless CEO, Ramona Faith, was was honored as one of seven Champions for Children by 4Cs of Sonoma County. Her passion for health and commitment to supporting children and families in our community has had a measurable impact in South County, and her family, staff, colleagues and Board of Directors are so proud of her! On November 1, PHCD was one of five recipients of the 2016 Jefferson Award for Public Service. Awarded by the Sonoma County Board of Supervisors but established nationally in 1972, the award is given to outstanding organizations and individuals who have had a deep impact on the quality of life in the communities they serve. Previous national recipients include Cesar Chavez, Jimmy Carter, Oprah Winfrey, and the Bill and Melinda Gates Foundation.
We are so proud to report that in Ocotober, PHCD was recognized as one of the Healthiest Companies in the North Bay! This is our third year receiving this recognition, and we are so proud of the work of PHCD staff who have been driving our Wellness Committee and Employee Club to bring fun and engaging opportunities to improve our health right here in our office!
